Pitt's
Conflict of Interest Form
-
Conflict
of Interest (COI) disclosures are due and must
be completed using the electronic form available
at the Conflict of Interest Superform site.
-
Completed
forms are due to Mary Ann Gebet not later than
April 1.
-
If you
completed the COI form electronically last year,
you may update the information submitted previously.
If you are using the electronic mode for the first
time, you will register when entering the website
by providing an e-mail address and a password.

When the
form has been completed, you will click on Record
my Data. You will then print a summary disclosure
form by clicking on Print My Form to File and
selecting the form from the Filing History screen.

Please
sign the summary disclosure form and provide it
to me for the review.

Please
complete and submit the form to Mary Ann Gebet
no later than April 1.
Pitt's
Prevention of Sexual Harrassment
- The
Provost has notified all Schools that the University
is committed to maintaining an environment in which
all can work and learn effectively with no fear
of harassment. To help achieve this goal,
all faculty and graduate teaching and research assistants
are required to complete the University 's online
course on the prevention of sexual harassment.
The course is available at http://www.newmedialearning.com/psh/pitt/.
We have been advised that you should allow about
45 minutes to complete the course. Once you complete
the course, please provide the Dean' s Office with
a signed certificate of completion, which we are
required to keep on file. The Dean must certify
to the Provost that all faculty have successfully
completed the course. It is important that
all faculty and graduate teaching and research assistants
complete this course. If you have any problems
in doing so, please let us know as soon as possible.
